Anxiety in Addiction Recovery: The Science Behind the Struggle
Anxiety and substance use disorders are deeply intertwined, so much so that researchers increasingly view them as a unified clinical entity rather than two separate conditions requiring sequential treatment. Population-based studies, including the landmark National Epidemiologic Survey on Alcohol and Related Conditions (NESARC), found that among individuals with substance use disorders, between 40 and 60 percent had a co-occurring anxiety disorder, rates dramatically higher than in the general population.

The seven items assess worry, inability to control worry, worry about multiple things, difficulty relaxing, restlessness, irritability, and fear that something awful might happen, all measured over the previous two weeks on a four-point frequency scale from "not at all" to "nearly every day." Total scores range from 0 to 21, with established severity thresholds at 5, 10, and 15.
Anxiety as a Relapse Driver
Anxiety is one of the most potent and consistent drivers of relapse across virtually all substance use categories. The mechanism is straightforward: substances temporarily suppress the amygdala-driven anxiety response, providing immediate relief that powerfully reinforces substance use as an anxiety management strategy. When anxiety re-emerges in recovery, as it inevitably does, often amplified by the neurological rebound of withdrawal, the learned urge to return to substances is activated with particular intensity.
This is precisely why anxiety management skills are not peripheral to recovery, they are central to it.
